New Patient Forms

Please see below for our new patient forms.

If you are not able to print our forms, please contact our office and other arrangements will be made for you. The main goal of the packet is to obtain a complete medical history and medical information from you as it pertains to your evaluation and treatment in our office. We realize this can take some time; however, completion of the medical history form is key to your doctor’s evaluation and diagnosis. Please allow yourself ample time to fill out the forms. They must be completed prior to the doctor seeing you.

Appointment Reminders

Approximately two business days before your appointment we will contact you to remind you of your upcoming visit. To allow us to accommodate the scheduling needs of our other patients, it is extremely important that you contact us a minimum of one business day prior to your appointment if you will not be able to keep your appointment and need to reschedule.

Your Initial Visit

Appointment Cancellation and Rescheduling Policy

Notification to reschedule or the cancellation of an appointment must be received by our office a minimum of one business day prior to your appointment or a $75.00 fee may be added to your account at our discretion.

Your first appointment will include a detailed medical history, physical examination and most likely, skin testing. Additional tests may be performed as deemed necessary by the doctor. If we are doing skin testing, you can expect your first appointment to last approximately one hour. Due to the length of your visit we recommend that young children do not accompany you to this appointment. Since many patients are being treated for allergies, we ask that you refrain from wearing perfume, cologne, scented lotions, etc., as they may trigger reactions in some patients. We thank you in advance for your cooperation.

Medications to Stop Prior to Skin Testing

Because some medications can impact allergy skin testing, we ask that you stop taking some medications 5 days prior to your appointment. Please click here for our Medications Guidelines for Skin Testing. If you feel that you aren’t able to do this for any reason, please contact us at 508-362-0099 for guidance. If you aren’t sure about any of medications please contact your pharmacy. If you have ANY questions about what medications you should stop prior to skin testing please call us. You should continue taking your other medications as prescribed. Do not stop taking any asthma medications or medication that treats blood pressure, heart problems, diabetes, or a psychological disorder. Do not stop using asthma inhalers!

Your Current Medications

We also ask you to bring a list of all of the current medications, vitamins, and/or herbal supplements you are taking to your first appointment. We know it can be difficult to remember everything you are taking, especially vitamins, supplements and eye drops that’s why we tell everyone to just scoop up all of their meds and supplements into a bag or box and bring them in for us. (Much easier than making a list!) This way we can document everything correctly and nothing gets missed.

Prescriptions and Refills

Please contact your pharmacy for refills. They will contact us, if needed, to complete your refill.

After Hours, On-Call

Our practice virtually always has a doctor on call in case of an emergency after our normal business hours. To reach an on-call doctor please call our main office number and your call will be forwarded.
